Planning and Teamwork Essential in Separation of Conjoined Twins: An Anesthesiology Perspective

Cerebral angiography, magnetic resonance imaging and computed tomography under general anesthesia were scheduled. Prior to the procedures, the anesthesia team met to discuss the anesthetic plan, anticipate problems and establish contingencies. By administering atropine to one twin and assessing the other twin’s cardiovascular response, we determined that the twins shared cerebral circulation and that drugs given to one twin would potentially affect the other. We evaluated each venue to determine how best to position equipment that would be duplicated (e.g., anesthesia machine, anesthesia cart, suction and monitors), and we color-coded all the monitoring cables, breathing circuits and intravenous lines to avoid the potential for confusion about what was going to whom.
